appropriate application title, click on the PDF file, then save the
file or print out the forms on your printer. After your forms
are filled in and completed, FAX them to our office for
review at 713-524-4306. We will begin the process of
submitting your application to the appropriate company for
approval.

contact the physician of your choice and verify that your insurance
is or is not accepted before you go (unless it is an emergency). If

whether or not you are in the correct network when verifying
